Falkirk project chosen as finalist in 2026 COSLA Excellence Awards
A collaborative Falkirk project that assists patients in returning quicker to their home after a hospital stay has made the shortlist for a prestigious award.
The Shifting the Balance initiative, led by Falkirk Health and Social Care Partnership (FHSCP), is amongst 15 finalists for the 2026 COSLA Excellence Awards chosen from entries submitted by local authorities in Scotland.
Launched last summer by Falkirk Council, the initiative is used as a method of streamlining the process of getting people back to their home as quickly and safely as possible.
Using a ‘Discharge to Assess’ model, people were provided support to recover at home while ongoing care requirements were assessed. As a result patients could relax in a more comforting environment, rather than recovering in a hospital bed.
Since the model was introduced, hospital stays have reduced significantly, with delayed discharge numbers having fallen to their lowest rate since April 2022.
The average length of stay for patients discharged dropped from 79 days in June, to 36 days in August, a decrease of more than half.
The dual initiative involves local care at home services, the Mobile Emergency Care Service, and a wide range of support from Home First, ReACH, District Nursing, and Allied Health Professionals.
Following a call for entries from all 32 of Scotland's local authorities, Shifting The Balance was shortlisted by an independent panel of industry experts. The entrant list highlights the best of Scottish local government across five categories, including innovation, tackling inequality, and strengthening communities.

Claire Chapman, interim head of community services, Falkirk Health and Social Care Partnership said: “Being recognised as a finalist is a fantastic achievement and reflects the commitment of colleagues across health, social care and the independent sector who worked together to make Shifting the Balance a success.
“By helping people return home sooner and supporting their recovery in familiar surroundings, we have improved outcomes for local residents while reducing pressure on hospital services. Most importantly, this approach has shown that with the right support in place, many people can regain independence and continue living well at home for longer.”
Winners will be announced at an awards ceremony at COSLA’s Annual Conference and Exhibition, on Thursday, November 19, in St Andrews.
